What TO EXPECT
Our referral Process
From the moment a referral is made, we walk alongside every young person and their network to ensure the transition into their new home is safe, supported and smooth.
1. Referral Received
We review the referral promptly and confirm whether we can meet the young person's needs. Referrals can be made by local authorities, social workers, or other professionals.
2. Initial Meeting
Our team meets with the young person and where appropriate, their social worker or support network to introduce ourselves, understand their story and begin building trust.
5. Placement Confirmation
Once all parties are satisfied and documentation is in place, the placement is formally confirmed and a move-in date agreed.
3. Home Tour
The young person is invited to visit and tour the home before any decisions are made. This is their chance to ask questions, get a feel for the environment and begin to picture themselves there.
6. Induction & Welcome
On arrival, the young person receives a full induction meeting the team, learning the rhythms of the home and understanding their rights and responsibilities. We make it a moment worth remembering: this is their home.
4. Risk Assessment & Care Planning
Working closely with the young person, their social worker and commissioning funder, we complete all relevant risk assessments and co-produce a care and support plan tailored to their individual needs and goals.
7. Settling In
In the first weeks, our team provides close support to help the young person adjust, feel safe and begin to thrive. Regular reviews ensure their plan continues to reflect their needs as they grow.
We don't just open our doors we make sure young people are ready and supported to walk through them.
Our homes currently welcome young people from the age of 18.
As we progress through regulatory approval, we look forward to extending that welcome to 16 and 17 year olds because the need for safe, quality care doesn't wait for adulthood.
